Property Status/Final Bill/Transfer Service Form

This form is for requesting a Status and Final Bill or Transfer of Service for properties in Lookout Mountain Water District. Title companies ordering a final bill or requesting status on a property, landlords requesting transfer of service into a new tenant's name, and tenants leaving a property must complete and submit this form. A final bill will be processed and emailed the day before closing date or the move date.  There is a $75 tap transfer fee included in the final bill for OWNER changes, such as a home sale, vs. merely a tenant change.
